Professional Activities
In Reconcept EPA Portfolio you keep track of your personal progress, experience and levels in various professional activities. What these professional activities are for you depends on the program you are following.
The ultimate goal is to be allowed to perform all professional activities independently and that your journey toward this is visible. Professional activities may include EPAs, skills or operations.
Do you have enough expecrience on a stated level? Then your supervisor may issue a higher level of competence. More information can be found at
Competence certificates
Request feedback
Feedback is requested using the Quick Actions button. You can choose from various feedback or assessment tools prepared by the program.
Learning activities
You record your various learning activities in the portfolio. This way, you collect data on your progress and make your own learning path and levels visible per topic. Your program provides feedback and assessment forms with which you can demonstrate your progress in a recognizable professional activity.
The collection of all completed feedback and assessment forms and all evidence you have provided yourself is called 'Learning Activities'. These learning activities might include formative assessments, interviews or multi-source feedback. Your program manages which types of activities are available to you as forms.
How do I add a learning activity to my portfolio?
To add a learning activity to your portfolio navigate to the blue Quick Actions button. Select your portfolio and choose the learning activity you want to add. Indicate who your supervisor is and link the relevant sections where you want the form to appear. Next, complete the form and then submit it to your supervisor. Your supervisor can complete the form and validate it with their approval.
Have you collected enough learning activities to demonstrate that you are ready for the next level? Then apply for a competence certificate!
Competence certificates
Have you gained enough experience for a particular professional activity and do you think you can move up a level? Then you can request a competence certificate from your supervisor*.
To request a competence certificate, follow the instructions below.
Use the Quick Actions button to go to My Portfolio and then to Competence Certificates
Select the desired competence certificate and supervisor
Choose the topic for which you want to request a competence certificate
Enter the level at which you want to request the competence certificate
If desired, you can add comments or attachments
You can choose between 'submit' and 'approve'. For direct approval, the supervisor has to enter the unique PIN code together with you.

The supervisor needs rights to issue formal assessments. Not all supervisors have these rights by default. For more information, see: Roles & Rights
Personal Goals & Residencies
In the section Personal Goals & Residencies, you have the space to further specify your own learning path.
You can add your own personal learning objectives - separate from the desired learning outcomes of your program.
You can link your learning objectives to relevant learning activities, discuss them during a meeting or add notes.
You can show your growth per learning objective.
You can add a target date to a learning objective and mark it as achieved when you have completed your learning objective.
Under Residencies, you can specify where you gain experience for each period. For example, you can add different teaching periods or residencies and the corresponding supervisors.
